Application Process

We are delighted that you have shown interest in our school. Below is an outline of the application process. Please contact us with any questions you may have.

Kindergarten and First Grade

1. Schedule a tour

Tours are an important part of the admissions process. Taking a school tour is a wonderful opportunity for families to see the facility, meet teachers and staff, see our students as they learn and ask questions along the way. To schedule your visit click here or call at 414-967-8315.

2. Send application

Return a completed application form to our office with a $100 non-refundable application fee. Make checks payable to MJDS. Children must be four years old by Sept. 1 to enter junior kindergarten and five years old by Sept. 1 to enter senior kindergarten.

3. Educator’s Report

Please give this form to your child’s current teacher or preschool program director, ensuring the person will have enough time to complete the questionnaire. Forms should be returned directly to MJDS.

4. Shadow morning

The admissions office will contact you to schedule a shadow morning. This is a wonderful way to get to know your child and for your child to get to know our school.

5. Skills Testing

We will contact you to schedule a time for your child to take the Bracken Basic Concepts Test. This is a simple assessment administered by the director of Curriculum and Instruction that takes no more than 30 minutes.

6. Notification of Admission

Upon completion of the admissions process, the applicant’s file is submitted to our Admissions Committee. A letter of admission is mailed out, which includes an Enrollment Agreement, to be completed and returned to the office by the indicated date, along with a copy of the child’s birth certificate.


Second through Eighth Grade

3. Records

The Authorization for Disclosure of Confidential Information authorizes MJDS to request your child’s “education records,” “pupil records” or other information as described in the form.

4. Student Visits

Student visits are a wonderful way to help us get to know your child and for your child to get to know our school. The admissions office will contact you to schedule a date for your child.

5. Notification of Admission
